Solarity Mental Health, PC can bill your primary insurance for face to face professional consultations. I am not a telehealth provider. Other services may not be covered by your insurance. Please check with your insurance carrier for your benefits.
Solarity Mental Health, PC will bill your secondary insurance if I am a paneled as a preferred provider.
The established procedure fees for services are:
$360.00 for initial basic psychiatric assessment (required to establish care)
$450.00 Psychiatric evaluation with medical complexity
Evaluation & Management (E&M) reviews are based time and complexity.
$100.00 min fee per encounter (10 min or less)
$5 Rebilling fee (not covered by your insuance)
Any prior approval from your insurance company will be your responsibility. Your copay may be a fixed amount set by your insurance company or it may be a percentage. Please be prepared to pay your co-pay at the time of your visit. If your insurance has a deductable amount you are responsible to pay that amount at the time of service. Your insurance company will still be billed to validate your deductable. Any amount not covered by your insurance will be bill to you. There will be a $5.00 rebilling fee for any balance that is not paid with in 30 days of the statement. Your insurance will not pay the fee for no shows, and they may not pay for telephone consultation.
Checks can be made payable to Solarity Mental Health, PC for any fees or co-pays due. Debit cards are also accepted. Credit transactions may incur a service fee.
There is a $30 fee for checks with insufficient funds.
Jolyn Zeller reserves the right to deny services, and may pursue collection for billed services.

Understanding your insurance benefits can be complicated. The following links are available to assist you in checking on your insurance benefits. Some companies require a preauthorization. If you are double covered, the primary coverage is based on the earliest birth date of the subscriber. Please be aware that both primary and secondary insurance may have a deductible.
Missed appointments or those cancelled less than 24 hours in advance will be charged$100.00 for a missed appointment fee. Be aware that insurance does not cover these fees. You will not be charged if there is an emergency situation or extreme circumstances, with my prior approval. This policy is strictly enforced.
